About The Providers:

Dr. Aadil Modi, MD

Dr. Modi got his undergraduate degree from the University of South Florida. He then completed his medical degree at Windsor University in Canada. He completed his Family Medicine Residency at Larkin Hospital in South Miami. Dr. Modi is Board Certified by the American Board of Family Medicine.

Dr. Modi chose family medicine for its focus on strong connections with patients and their families. He enjoys being able to develop personalized care plans tailored to each individual’s needs. Rooted in evidence-based practice, Family Medicine follows the latest guidelines and research. It places a strong emphasis on preventative care to support long-term health and well- being.

When Dr. Modi is not seeing patients, he enjoys being outdoors with his wife and three kids. They like biking, swimming, and playing basketball together. Dr. Aadil Modi was born and raised in Florida. He has always enjoyed an active lifestyle and loves the sunshine.

Jocelyn Perez, APRN, FNP-C

Jocelyn Perez is a certified nurse practitioner. She earned her Bachelor of Science in Nursing from Grand Canyon University in Phoenix, Arizona. She earned her master’s degree in Family Practice from Miami Regional University. There, she became an Advanced Practice Registered Nurse (APRN).

She is a member of the American Association of Nurse Practitioners and the International Honor Society of Nursing. She is also part of Sigma Theta Tau.

Jocelyn has a variety of experiences with different specialties. She started her career as a cardiac registered nurse working in a pulmonary specialty group. There, she cared for patients in different settings like the ICU, hospital, rehab, and outpatient clinics.

A healthcare diagnosis her father received led her to this profession. As she studied medicine, she noticed that older adults often face similar problems as her father. They may delay care because of costs, limited access, or not understanding their health needs.

Caring for seniors helps her tackle these issues directly. This support allows them to keep their dignity, independence, and quality of life. Her goal is to provide compassionate care that ensures they receive the attention and support they deserve.

Mrs. Perez met her husband in the medical field. Their love for medicine and helping others drives them to keep working. This has built a strong base for their future.

Yes! Curana Health bills insurance just like a regular doctor’s office.

Of course! Curana Health Clinic providers are available to see you whether we are your primary doctor or not. However, you are more than welcome to switch to us as your primary care physician. We would love to have the opportunity to serve you.

The number of residents that sign up determines our providers’ schedules. The minimum is one day a week with five days a week being the maximum. We are available 24/7 by phone call.

We provide many services. These include blood pressure checks, ear cleanings, lab draws, and therapeutic injections. We also help with medication management and refills, along with other services.

Yes! It is what we do. Our goal is to provide complete care for our patients. We work with any doctors or specialists you see, ensuring that you get the best care possible

Yes. Our providers will send notes to all doctors, including your primary care doctor. This keeps everyone updated on your care.
